I, the undersigned parent or legal guardian of the above-named athlete, acknowledge that participation in wrestling includes physical contact and involves the risk of serious injury, including but not limited to sprains, strains, concussions, broken bones, or in rare cases, permanent disability or death.

I affirm that the athlete is in good health and capable of participating in all wrestling activities.

I agree to be responsible for any and all medical expenses that may be incurred as a result of injury during participation.

I assume all risks associated with participation in wrestling practices, training, tournaments, and travel to and from such events.

I release and discharge the Wrestling Club, Hillcrest High School, Alvord Unified School District, its coaches, volunteers, staff, affiliates, and sponsoring organizations from any and all liability, claims, demands, or actions that may arise out of or relate to any injury, illness, or loss incurred during participation
